Your Attendance Allowance questionnaire

This takes most people 30–45 minutes. You can't get anything "wrong", and you don't have to finish in one go. Answer about your bad days as well as your good days — the DWP needs to understand your life when it's hardest.

The most important thing to remember: if you've found ways to cope — a stick, a rail, a routine, or a wife, husband, family member or neighbour who helps or checks on you — you STILL have the difficulty. Coping doesn't mean it isn't there. Tick it, and tell us what would happen without that help or aid.
How it works: for each part of daily life you'll tick what you find difficult, then how you manage. Underneath, you'll see the answers we've written from your ticks — you can change the words if you like. Anything that doesn't fit goes into "extra detail" and we include it in your application, so nothing you tell us is wasted.

About you

This is the person the application is for.

Your doctors and hospitals

Why this matters: the DWP may contact whoever knows you best. A GP who rarely sees you is less use than a consultant, community nurse or specialist who knows exactly how you are — so tell us about all of them.
If you see a different doctor every time, just put "varies" — that's completely normal and it doesn't harm the claim.

Your circumstances

Why we ask: these affect how the DWP handles your claim, and some can speed it up considerably.
If yes, there are "special rules" that get claims decided in days rather than months, at the highest rate, without the usual questions. Please tell us and we'll handle it sensitively and urgently.
If yes, they may be entitled to Carer's Allowance in their own right — we'll let you know when we review your answers.

Your health conditions

Don't leave anything out. Tick everything, even things that feel minor or "just old age" — conditions add up, and together they paint the true picture. Include anything a doctor has ever mentioned.

Anything else?

Anything about your daily life, health, or the help you need that we haven't asked? Bad days, hospital stays, carers who visit, things you've had to give up — it all helps your claim.

Send your answers

Press send and your answers come straight to us. We'll check everything, write up your full application, and be in touch — usually within one working day. Nothing goes to the DWP until you've approved it.
